Community Connect

Community Connect serves as a one-stop entry point for pregnant women, families, caregivers, and individuals in North Dakota, providing connection to essential services including housing, employment, financial support, parenting resources, physical health, and community connections.

Services Provided

  • Care Coordination - a care coordinator assists with short and long-term goals, barriers, and provides a source of connection and support
  • Peer Support - a peer support specialist with similar lived experience offers hope, support, and advocacy
  • Recovery Services - access to services or programs to assist individuals with meeting their needs and goals

Eligibility Criteria

  • 18 years of age or older
  • Resides in North Dakota
  • Has a Mental Health or Substance Use Disorder diagnosis impacting functionality in multiple domains including housing, employment, parenting, physical health, or community connections
