Targeted Case Management
Targeted Case Management (TCM) services assist individuals with serious mental illness (SMI) or serious emotional disturbance (SED) in gaining access to needed medical, social, educational, and other services necessary for appropriate care and treatment.
Services Provided
- Comprehensive assessment and periodic reassessment of individual needs
- Development and periodic revision of a specific care plan based on assessed needs
- Referral and related activities to help link individuals with medical, social, educational providers, and other programs
- Monitoring and follow-up activities to ensure the care plan is implemented and adequately addresses needs
- Scheduling appointments and coordinating with service providers
- Working with individuals and their authorized health care decision makers to develop goals
Eligibility Criteria
- Individuals with a serious mental illness (SMI) diagnosis
- Children and adolescents with serious emotional disturbance (SED)
- Must be eligible under the North Dakota Medicaid State Plan
- Requires documented need for case management services
